Floatation Therapy Liability Waiver

You'll be emailed this waiver to complete and sign before your appointment.

This is the liability waiver for Floatation Therapy at Float Therapy Wilmslow. Once you've made a booking, we'll email this waiver to you to complete and sign before you attend your appointment. We've published it here too, so you know exactly what to expect.

Please read each section carefully. If anything is unclear, please ask a member of the Float Therapy Wilmslow team before your appointment.

Before you float

I understand that I will be spending one hour alone, floating in approximately 12 inches of water.

I am able to climb in and out of the float pod unaided, and I am able to open and close the pod lid myself.

Protecting the float tank

Important: If any pollutants enter the floatation tank as a result of hair colourant, tattoo ink, false tan, or similar products, a fee of £900.00 will be charged to refill the tank with water and 550kg of Epsom salts.

Health conditions

I will not use the floatation tank under the influence of alcohol or drugs.

I am not currently suffering from any form of ear infection that may be made worse by floating.

I do not have any fresh tattoos, meaning tattoos completed within the last 2 weeks.

I do not have recently dyed hair or recently applied false tan where the water is not yet running clear in the shower.

I understand that salt water can affect the glue used to attach hair extensions.

I am not over 35 weeks pregnant. Unfortunately, we are unable to facilitate floating beyond 35 weeks due to the proximity of birth.

I am over the age of 16.

If you have a mental health condition that may affect your ability to float safely or comfortably, such as PTSD, schizophrenia, severe depression, or suicidal thoughts, we recommend that you seek approval from a medical practitioner before floating.

If you have a physical health condition that may affect your ability to float safely or comfortably, such as epilepsy, a heart condition, asthma, a skin condition, allergies (particularly to salt or magnesium), or very low blood pressure, we recommend that you seek approval from a medical practitioner before floating.

Allergies and reactions

I understand that the float pod water solution contains Epsom salt and H2O2.

I understand that oxygen-based oxidisers are used to treat the water and destroy bacteria.

I acknowledge that, in rare cases, some people may experience skin allergies or reactions to these chemicals.

If any of the above information changes, it is my responsibility to inform Float Therapy Ltd before floating.

Health disclosure

When you complete the waiver, you'll be asked to tell us about any relevant health conditions, allergies, injuries, or anything else we should be aware of before your float.

Late arrival and no-show policy

Please arrive on time so that you can receive your full appointment.

If you arrive more than 10 minutes late for your appointment without prior notice, your appointment will be treated as a no-show. Any credit used for the appointment will be lost and will be non-refundable.

Client declaration

By submitting the waiver, you confirm: “I have read and agree to the terms above. I confirm that the information I have provided is accurate, and I understand that it is my responsibility to tell Float Therapy Ltd if anything changes before I float.”

You'll provide your full name and the date. There is also an optional marketing preferences section, which is separate from your treatment waiver.
